Every field, check and condition on this page is taken from Hoshen's hot work procedure. No check has been added that is not in the procedure.

Generating this form does not authorise the work. The permit is valid only after a check on site and the signature of a qualified approver on behalf of the site, and only for the work, location, time and team approved on the form. This tool fills in the procedure's template and nothing more.

Conditions from the procedure

This form is a generic template. At a site with a binding permit-to-work system, the site's own form must be used, with the specific requirements added according to the risk.
